We are pleased to be able to offer a full-time band 4 Multi-therapy Assistant role working in the Wokingham area of Berkshire. We are an integrated community team including Occupational Therapists, Physiotherapists, and Therapy assistants. The service supports multiple pathways including Community Reablement, Rapid Response, Falls and Community Physiotherapy. This is an exciting time to join BHFT, as we develop services to work in an integrated way within the community in Wokingham and across Berkshire West.
- Support all intermediate care pathways to promote the timely assessment and management of referrals with multiple co-morbidities and rehabilitation needs.
- To support band 3 therapy assistants of the Intermediate care team with clinical development.
- To support the allocation of visits for the therapy assistant team
- To support the triage of referrals into the intermediate care team reviewing service user needs and sign posting to other services when indicated.
- Provision of equipment to promote independence and safety.
- Hold a caseload and work autonomously under the guidance and support of the therapy assistant coordinator and therapists.
- Work flexibly to ensure urgent referrals are adequately managed
- Supporting colleagues in the delivery of compassionate, safe and structured care
- Provide guidance and appropriate delegation within the team to band 3 colleagues.
- Participate in service development and quality improvement

2. Current understanding of intermediate care with experience of rehabilitation and hospital discharge
3. Car driver with ability and readiness to be flexible within working day

Working pattern + Service operating hours
Intermediate care team is a 7-day service which operates from 08.00 - 22.00hrs with Early (8:00-16:00) and Late shifts (14:00-22:00). You would be expected to be able to cover all aspects of the shift pattern. Shifts are rostered 3 months in advance and shifts/days off etc can be requested in advance. Flexible working requests can be discussed and considered at interview.

Parking permits provided for nearby public car park, however remote working is often encouraged when appropriate to do so.
