6 Months - Current NHS employer -secondoment only Non NHS - Fixed term 2 role (60hrs between both roles) interviews to take place 8th August The post holder will be responsible for providing support to the York Place Single Discharge Coordinator(commissioning support) and project delivery to support the ICB to achieve timely, safe and best value discharge processes from York Acute site and York Community hospitals, this may include supporting transformational change and quality improvement through the implementation of significant integration and service redesign projects. These will address key system priorities, including proactive services to support the system including the interface with UCR, admission avoidance, Early discharge process and virtual ward development (frail people, urgent crisis response services, virtual wards, and bringing together reablement and rehabilitation) services to support hospital discharges. This will be achieved by: Leading on delegated specific service improvement projects to develop discharge processes through (commission) high quality services that best meet the needs of the local population and is based on sound discharge data and population health management data. Leading on delegated specific improvement projects focused on improving quality of discharges of those with no criteria to reside in a timely manner (the ambition of discharge on same day for all but the most complex clients).

Responding to the data monitoring of NHS England, Community Collaborative and internal Trust data on discharges with regard to length of stay and report to the Single Discharge Coordinator and take action as delegated/accordingly. Providing a high-level focus on delivery, working within agreed protocols, underpinned by best practice and with recognised tools and methodologies. Providing high quality support that is timely, responsive and fit for purpose. Providing expertise as required on services in steady state, transitioned and those ready to be commissioned; including activities to inform a commissioning decision.
