Salary £24,054-£27,852 Per annum | Permanent, Full Time, 37 hours per week | Urgent Care and Health Service | **Social Care Support Officers** **Urgent Care and Health Service** **Countywide, Lancashire** **Including - Central Lancashire, West Lancashire, East Lancashire, Fylde and Wyre and Lancaster and Morecambe Bay.** **We are currently recruiting ** Motivated and decisive **Social Care Support Officers **to work within the **Urgent Care and Health Service **in **Adult Social Care **at **Lancashire County Council.** Our teams are committed and focused to avoid hospital admission and to ensure safe and timely discharge for our service users wherever possible, acting swiftly to assess for the necessary care. As our new Social Care Support Officer, you will be working closely with a multitude of services to ensure the best possible outcomes for individuals in hospital settings, care homes, and at home. This environment is fast paced which requires diligence and quick reactions in all situations, aiming to improve the lives of all our service users through creative and evidence-based practice. All posts are 7 day working, including 1 in 4 weekends, which also covers 8-6pm with a view of 8-8am in the future, unless specified. **What we are looking for ** **Passion **- you'll be passionate about making a difference to people's lives, determining the right level of care and providing superior holistic assessments - we want you to be autonomous and an advocate for our service users, forming a worthwhile connection with your calm and collected demeanour - you will be supporting with early discharges from hospital as well as visiting people's homes to assess various situations, so we need you to be courteous and respectful in every setting - your attention to detail will be second to none, identifying any and all risks to prevent hospital admission and mitigate these risks for hospital discharge.
