Pixel Personnel are currently recruiting for an experienced Social Worker for a Transfer of Care Team based in Cambridge. Excellent rates of pay on offer up to £38 per hour (Umbrella). This is a full time role based on 37 hours per week. Part of the rolemay involve visiting CQC inspected Care Homes Hybrid working arrangement on offer, flexibility is required to carry out face to face assessments in a hospital and or community setting, excellent support available from the Management Team. Social Worker Transferof Care Duties: Posts are for 3 months funding from the start date of the Social Worker to carry out assessments (currently being carried out virtually via all methods of tech: skype, Teams, telephone etc) which have to be completed as soon as possible postdischarge.This is called 'Discharge to Assess'. The successful Social Worker will need to be able to communicate effectively with people and their support networks where appropriate and to be able to deal with complex issues professionally and timely and tomeet targets for completion and transfers to locality teams as required.
