Are you an experienced, dynamic and committed learning disability nurse who wants to be able to make a difference to the health outcomes for people with learning disabilities? We are looking for a passionate individual with experience of working with stakeholders and promoting the health needs for the learning disability population.

The role will involve working closely with GP practices to ensure annual health checks are carried out and to a good standard. There will be the opportunity to support in the coproduction of training to support GP practice staff to be aware of the needs of people with learning disability.

The role will involve travelling to GP practices within the county and building positive working relationships with GP practices and other stakeholders involved in the care of people with learning disabilities.

The post holder will be required to collect and collate a range of data from primary care services that can be used to develop local strategies for improving the health care of people with learning disabilities. This will include conducting audits on the quality of Annual Health Checks.

The post holder will primarily be involved in supporting Primary Care to improve access to their services with the aim of reducing health inequalities, reducing avoidable illness and preventing premature deaths of people with learning disabilities in Northamptonshire.

They will work alongside GP practices and support them to identify and implement strategies to reduce health inequalities and make reasonable adjustments to their services to ensure equity of access for people with learning disabilities. This will include promotion of the Learning Disability Annual Health Check (AHC).

They will provide liaison and advice to Primary Care regarding their patients with learning disabilities and seek to identify solutions and appropriate sign posting. They will also work alongside people with learning disabilities, carers and families to support and facilitate effective engagement with primary care services.

The post holder will be required to contribute to the development and the delivery of training sessions for a range of services regarding the health needs of people with learning disabilities. They will provide advice and signposting as required to a range of health and social care services.

People with learning disabilities are at the forefront of what we do within the transforming care team within Northamptonshire ICB. We work closely with a number of stakeholders involved in improving the health outcomes for this population.

As part of the ICB, you will be able to make a real difference to the lives and wellbeing of Northamptonshire's learning disability population.

1. Core Duties and Responsibilities (Key Result Areas)
- To support GP practices to identify their patients who have learning disabilities.
- To assist the practices in stratifying their Learning disability registers.
- Support GP practices to meet the Learning Disabilities Health Check Scheme
- Enhanced Service requirements within the specified time frame.
- Monitor the uptake of the Annual health checks at the GP practices from the data received from NHS England. Provide support and specialist advice to those practices with low uptake.
- To assist GP practices with the development Health Action Planning.
- To support the development of Learning Disability registers, ‘flagging’ systems and recognition of learning disability by Health Services.
- Develop own knowledge in health inequalities of people with Learning Disabilities and local health issues and share this knowledge with Primary care and act as a resource for staff.
- Adapt local training packages to reflect current research and national drivers.
- To advise and support healthcare services to make reasonable adjustments for their learning disabled population.
- Provide specialist advice and support to other health and social care professionals inlinewith best evidence based practice.
- To advise organisations on healthcare needs of adults with learning disabilities,
- To provide Learning Disability health care needs training to a variety of services.
- To engage with individuals, families and carers in consultation exercises and events relating to healthcare and wellbeing.
- Promote equal access and benefits from mainstream health & wellbeing services.
- To raise awareness of the health needs of people with learning disabilities by identifying and meeting training needs through both the delivery of training and by signposting carers and workers towards appropriate services.
- Lead local improvements regarding health information for people with a learning disability and their families. This may include developing and enhancing local pathways through services.
- To provide specialist advice relating to people with learning disabilities, across specialist services, primary & secondary care.
