This is an exciting opportunity to join a team of Family Help 0-5 Practitioners operating as part of a targeted Family Help Service within the Chesterfield locality.

You will be responsible for supporting the Family Help Senior Practitioner in the efficient and effective delivery of 0-5 years Family Help Services demonstrating and championing evidence based systemic practice.

Your principal role will include:

- Compiling action plans with measurable targets to co-ordinate support for families who are working with a variety of partner agencies.
- Tracking the individual progress of a child and cohorts of children where appropriate, measuring development and school readiness.
- Deliver specific intervention programs and evidenced based programs targeted towards children at risk of developmental delay such as Every child a talker (ECaT) program.
- Delivery of programs to assist parents in their parenting, such as the Solihull approach, understanding your child.
- Providing advice and signposting to parents/carers.
- Communicating and building effective relationships with partner agencies such as Health and Social Care.
- Keeping clear, concise, and accurate records on the councils recording systems.
- Report writing and completion of key documents such as safeguarding case conference reports to assist in collating essential evidence gathering to ensure decisions can be made to keep children safe.
