Community Dementia Support Worker

This is an exciting new opportunity to work with our Primary Care Dementia Practitioner Service. This is a countywide motivated innovative team who work closely with General Practitioners, community services and the third sector. We support people after their diagnosis and help people with dementia and their families live well. We offer support advise and help resolve any problems they may experience. We are always there when they need us.

The initial posts are as follows:
3 x Band 3 Community Dementia Support Workers to deliver;
- APHC (using remote health checks)
- Monitoring role and referral/support with PCDP
- Improving recording of health checks and dementia diagnosis
- Case scenario outcomes to feed into service re-design

Working and based in the following areas;

1 x Falmouth

1 x St.Austell

1 x Bodmin
