**The role of a Care Coordinator**

The Care Coordinator is responsible for coordinating our care rosters each week, to ensure the continuity of our high quality services to those who need them. Your duties will include (but not limited to):

- Creating rosters to meet the needs of our clients and team members
- Coordinating any changes in services required, including but not limited to, increases in services, decreases in services, time changes, cancelled calls, etc.
- Booking in new clients, establishing times, and communicating to all persons involved, including social services, clients, client’s families and/or representatives and team members
- Forwarding key information to relevant personnel regarding changes in care
- Implementing and managing new systems
- Reviewing, monitoring, and managing staff annual leave entitlements, ensuring the business has sufficient capacity to deliver all care visits

But overcoming these challenges can be so rewarding and satisfying.
