The post holder will work within the Crisis Resolution and Home Treatment Team for Older People (CRHTT-OP North) in Peterborough which includes the provision of Dementia Intensive Support (DIST) for people who live with challenging behavioural and psychological symptoms of dementia.
The team cares for people who live in the Peterborough, Huntingdon and Fenland areas and the post holder will be required to work in a variety of locations such as nursing / residential homes and the patient’s own home.
The post holder will be an experienced practitioner required to take an active role in implementing behavioural management techniques for people who live with dementia and be able to demonstrate those techniques to both paid and unpaid carers. Experience of supporting people and their carers living with dementia is essential.
The post holder will manage delegated tasks for several service users, whilst under the supervision of a Band 5 / 6 Mental health nurse/practitioner or above.
You will be supporting the team’s CPNs by attending Initial Assessments for patients who are experiencing Behavioural and psychological symptoms of dementia. Patients will be seen in care home environments as well as in their own homes.
You will work closely with carers and family and help identify what will help the person and their carers to live well with dementia. You will work closely with care coordinators as well as working independently offering regular intensive care support, identifying triggers to expressed behaviours, role modelling, and psycho education. You will also be responsible for carrying out relevant physical health observations.

Assist in the delivery of a high standard of person-centred care to service users and promoting their dignity and equality at all times.
Implement therapeutic activity and other interventions as planned by the team.
Ensuring that all duties are carried out to the highest possible standard and are in accordance with current quality initiatives.
Report to the team any progress or changes in the service user’s health, care or needs.
Record interventions correctly and in line with legislation, policy and procedure.
Assist service users to maintain personal care and maintain their right to privacy, dignity and respect at all times.
Take appropriate action to manage any emergency, summoning assistance when necessary.
Report actual or potential problems to the. Care Co-ordinator or Team Manager
Establish and maintain professionally collaborative relationships with nursing / residential homes, JET, ICT and other professional groups.
Pass on constructive views and ideas for improving services for users and carers.
Undertake tasks as directed by the team practitioners, and MDT
Raise and report any safeguarding issues to the multidisciplinary team and safeguarding lead.
Understand the need to obtain consent and the process of obtaining consent.
