To work as a member of the Urology cancer nursing team providing patient coordination and care, for patients with cancer.

Provide coordination of high quality patient care through on going telephonic and proactive identification of needs utilizing basic clinical knowledge, approved tools and procedures.

Collaborate and communicate with the CNS and wider Health Care Team to ensure the provision of appropriate services to maximize the health and quality of life of the patient, improving the quality and efficiency of health care delivery.

Document and monitor all aspects of patient care coordination and service delivery, supporting data collection for audit.

Coordinate care needs of patientsusing the Macmillan holistic needs assessment

Triage incoming calls and initiate appropriate or sign posting when appropriate.

Utilizing good communication

Promote patient involvement relating to the provision of cancer care

Maintain supply of stationary and effective filling systems

Maintaining a supply of patients information booklets/leaflets

To contribute to an expertspecializedservice in Urology, that positively impacts on the experience of patients and carers by providing high standards of clinical practice and care

Provides a recognizable Macmillan experience by ensuring people living with cancer are aware they are interacting with a Macmillan Professional, and know the full range of resources and services available through Macmillan, navigating and coordinating where necessary.

Works in collaboration with services across the cancer pathway, to implement plans and to monitor their effectiveness in improving delivery and meeting the needs of people living with cancer.

Leads by example, contributes to the delivery of specialist education and training to other professionals involved in patient care; promotes best practice, supports and coaches less experienced colleagues and non-cancer specialists to achieve high quality, personalized care for people living with cancer.
