The Long Term Conditions (LTCs) team works with Primary, Secondary and Community care providers to identify new cases and optimise people with existing LTCs,
Working in this team will provide an opportunity to work with a broad range of service providers, to work on long term programmes with measurable outcomes, to manage a complex range of tasks and offers an opportunity for personal and professional development.
**Key activities will include**:
Planning, organising, managing projects, data analysis, information management reporting, responding to queries and communication with a range of colleagues and service sectors.
The LLR ICB is the NHS organisation responsible for planning, arranging and ensuring the availability and quality of health services to local people..
**The ICB’s functions include**:
- Planning to meet health and healthcare needs of our population;
- Responsibility for allocating NHS resources (budgets) to deliver across LLR;
- Ensuring that local NHS organisations are working well together, as well as with local government, and establish join working arrangements with partners
- Holding the NHS organisations in LLR to account for the services they provide;
- Leading local plans for developing key NHS resource;
- Ensuring the availability and optimum use of health data relating to the LLR population

The post holder would manage a portfolio of work, both supporting colleagues and delivering their own work.
**This will include**:
Working with LTC colleagues to preparing reports for committees, steering groups and boards to ensure that the LTC team presents timely and accurate information.
Developing presentations to explain complex information and demonstrate the team's progress against target
Managing a calendar of deadlines for reporting and assurance and making sure that colleagues are on track
Planning and supporting meetings
Contributing to service development and improvment
