**Summary**
The role of practice lead support worker involves remaining in an allocated service in a support worker role. The service is based in Whalley Range. You will need to be flexible and adaptable as your allocated service may change based on the needs of the service and people supported across the organisation.
In addition to the support worker role, you will work alongside the management team and the Central Support Team to support the implementation of the PBS framework across the service. Your roles and responsibilities will remain that of a support worker, with additions responsibilities listed below.
**Responsibilities**
- Support in accessing activities including hiking, swimming, horse riding, canal boating, dog walking, arts & crafts, music, drama and socialising with others.
- Support individuals to engage in social, recreational, and educational activities.
- Promote independence and encourage decision-making in all aspects of life.
- Build strong, trusting relationships with the people we support, their families, and other professionals.
- Ensure accurate records are maintained.
- Work in-line with care plans.
- Assist with daily living activities, including personal care, meal preparation, and household tasks.
- You will act as culture champion for your allocated service, this will involve leading a short segment within team meetings to inform and educate your team about City Care’s values. You will also take a lead on ensuring that your service ethos is updated as and when required, with input from the team and people you support (where possible).
- You will be responsible for monitoring paperwork and recording systems which are put in place by the Central Support Team or Redstone PBS. You will ensure they are completed as required by the staff team and assist staff members where additional support is needed.
- You will be responsible for carrying out monthly Periodic Service Review (PSR), you will do this alongside another support worker within the team to help educate others on the process and benefits of PSR. You communicate areas of success and areas of development to the team, team leader and PBS practitioner on a monthly basis. You will be involved on working on making improvements where appropriate.
- You will take an active role in tenants voice systems and debrief systems for the people you support within your service, ensuring that these tools are used for/with the people you support in accordance with their support plans.
- You will maintain a positive working attitude, striving to instil this in others you are working with and promoting the values of PBS. You may attend meetings with the management team and/or Central Support Team regarding the people you support to help generate ideas, monitor and evaluate interventions.
- You will complete PBS Level 3 with support from an internal mentor. You will demonstrate good progress and the ability to work to targets and deadlines.
**Requirements**:
- Proven experience in the social care sector.
- The ability to motivate and manage a team effectively.
- Excellent communication and interpersonal skills.
- Relevant qualifications in health and social care or related fields are preferred.
Pay: £27,873.00 per year
