In this role, you’ll co-design and deliver collaborative care plans alongside experienced colleagues, supporting people on their journey to recovery. You’ll provide both practical and emotional support, empowering individuals to engage with their local communities and live fulfilling, meaningful lives.
**Your responsibilities will include**:
Carrying out physical health assessments and monitoring, including phlebotomy duties and has to be phlebotomist trained and able to work under pressure
Supporting safety planning and participating in multi-agency meetings to ensure coordinated, high-quality care.
Facilitating access to a wide range of services that promote wellbeing and independence.

**Your Main Duties Will Include**:

- Providing practical and emotional support to individuals throughout their recovery journey.
- Conducting physical health assessments, including phlebotomy and monitoring vital signs.
- Supporting safety planning and contributing to multi-agency discussions to ensure coordinated care.
- Encouraging and facilitating engagement with local community resources and services.
- Conduct home visits where necessary using own transport or public transport with the use of a Lone working device and safe portable bag provided by the Trust
- Promoting wellbeing and empowering individuals to live meaningful, independent lives.
