Our client provides high quality palliative and end of life care to a significant and growing number of people living with a life limiting illness in Northeast Wales and beyond.
The organisation is seeking an experienced qualified Social Worker to join the Family Support Team. You will be an effective communicator with effective interpersonal skills and have the following:
- Experienced in Palliative Care Social Work or Previous experience of direct work with people experiencing emotional, personal, or practical problems and a desire to develop the required skills and knowledge
- Experience of working within a multi-disciplinary team
- Assessment skills suitable for working with all age groups
- Teaching Skills and experience in preparation and delivery of education
- Experience supporting students (practice educator)
You will need to have excellent organisational skills and be pro-active in all aspects of your role while working collaboratively with other professionals to support and empower those accessing services to make positive change.
Your passion to work holistically with those accessing services, along with your understanding of the social, psychological and spiritual needs of individuals and families facing life limiting illness, death and bereavement will ensure your ability to effectivelysupport and empower others.
As a Social Worker in The Family Support team you will also provide non-medicalised bereavement support to children accessing our Release service and pre and post bereavement support to those
connected with hospice services.
