The post holder will work as part of the Community Integrated Assessment Team to provide high quality rehabilitation care and support to people who are referred to the Service.
To assist and work under the direction of the Therapy Lead, individual therapists, and other senior members of CIAT where relevant and appropriate to deliver individual rehabilitation and care programmes both within the residential rehabilitation unit or in people’s own homes.
Making every contact count providing rehabilitation and equipment to patients to empower them to live well at home.
We are excited to offer an opportunity for a band 3 member of staff to join the Community Integrated Assessment Team, to work within the Integrated Intermediate Care team. The post is a permanent position and will be a full-time post.
You will be expected to work flexibly and available for both extended day and seven day working. In return we will provide you with robust induction support and the opportunity to develop with the guidance of experienced Senior staff. We actively support CPD. We pride ourselves on the friendliness of our service.
