We have a career enhancing opportunity for you to work as an Independence Coordinator.
We offer the foundations of a career where you can make a real difference to adults with care and support needs in the local area.
You will be working alongside a wider multidisciplinary team which includes Social Workers/Social Care Assessors and Occupational Therapists, with a clear focus on Reablement.
As part of the Reablement team you will be responsible for preventing, reducing, and delaying the needs of the adults with care and support needs and their carers. Working with people in the community and hospitals you will provide simple assessment advice, information, and guidance around non-complex equipment, adaptations, technology and telecare solutions, and making referrals to the community teams where appropriate. You will complete simple assessments looking at the care and support needs of the person. Where appropriate you will offer the Reablement service to enable the person to meet their agreed outcomes and goals by promoting and maximising independence, and again looking at equipment and assistive technology to support this. Information gathered will be recorded within LiquidLogic (LLA), ensuring this is accurate and timely.
You will have a minimum of a Level 2 in Health and Social Care, or QCF Level 2 diploma in Health and Social Care, with the service committed to give you the ongoing support and training to achieve Level 3 to aid your professional development.
Enhanced Level DBS Disclosure is required for appointment to this post.
