CERT was set up in 2014 to provide intensive, bespoke packages of care to service users in their own homes, as an alternative to being in locked rehabilitation, out of city hospital placements. The team works with service users with mental health problems, personality disorders and/or learning disabilities to support them to move from hospital into their own tenancies. Referrals are primarily received from inpatient acute and rehabilitation services.
Service users are supported by a full multi-disciplinary team with the aim of enabling them to live independent and fulfilled lives. The team provides intensive quality interventions between 8am and 8pm, 7 days a week.
As a Key Worker you will be responsible for managing a small, but intensive caseload of service users and supervising a mini team of recovery workers. You will also be expected to provide leadership and coordination to the team on a day to day basis.
The post holder will work within a multi disciplinary team to work with people to identify their own recovery goals and take back control of their own lives in order to live as independently as possible. As a Band 6 Key Worker within the team you will be responsible for the care coordination of a small number of people and the organisation and supervision of a mini team of recovery workers.
You will also be responsible for coordinating the day and responding to crises where necessary, delegating tasks to colleagues.

Clinical
Demonstrate a thorough and impassioned understanding of Recovery
Evidence the implementation of Recovery Focussed practice
Responsible for the collaborative assessment, planning and delivery of recovery focussed goals including developing strategies in regard to positive risk taking
Responsible for coordinating crisis management
Ensure delivery of recovery focused interventions which include the provision of recovery based activity.
Encourage creativity in recovery planning within the processes of the Mental Health Case Framework
Undertake assessments of people referred to the services
Coordinate in-reach to referring services to identify early goals and relationship building
Develop graded approaches to discharge from hospital in conjunction with existing Multi-Disciplinary Teams
Build positive collaborative recovery relationships within professional boundaries
Deliver quality interventions appropriate to recovery needs using Psycho-social Interventions/Cognitive Behavioural Therapy informed practice.
Monitor and evaluate risk through the use of tools such as the Detailed Risk Assessment and Management Plan (DRAM)
Creation of relapse prevention plans which identify early warning signs and triggers to ensure timely and proactive response
Complete relevant assessment tools and outcome measures including clustering
Be actively involved in clinical review including Multi-Disciplinary Team meetings
Complete carers assessments and support families, referring to family intervention services where appropriate
Complete referrals to social care.
Follow policy and procedure to ensure safe storage of medication (nursing).
Administer and monitor medication prescribed to people providing information and advice (nursing).
Education & Training
Undertake mandatory and appropriate training relevant to your role
Identify own training and developmental needs
Provide cascade learning of recovery focused practice and other expertise skills such as psycho social interventions
Assist in the delivery of training in areas of personal experience and interest
Prepare reflections of positive working or difficult situations
Supervision
Supervise recovery workers according to activities and duties identified within recovery plans, these could include the following:
Identify their own goals to improve their quality of life
Write their own recovery plans
To develop recovery plans with their care coordinator and implement them creatively using initiative
