Personalisation Support Worker

Focus Birmingham are a passionate local charity dedicated to empowering individuals with sight loss and complex care needs. We're looking for a **Personalisation Support Worker** to make a meaningful impact in the lives of those we support and their families.
In this role, you'll provide tailored care, ensuring that our support planning and activity programs are delivered to a high standard and meet the diverse needs of those we support. Each day at our vibrant Day Service is unique, so we’re looking for energetic individuals who can address both the physical and emotional needs of everyone in our care.
**You will lead our person-centered activity program by**:
- Planning and Leading Activities: Design and conduct activities tailored to participants across the service, promoting active support.
- Collaborating with Senior Support Workers: Facilitate meaningful activities that enhance community access, skill-building, and independence.
- Supporting Staff and Volunteers: Guide care support workers, agency staff, and volunteers to ensure active involvement for all.
- Modifying activities for individual Needs: Tailor activities to accommodate diverse abilities, using tools like assistive technology, environmental adjustments, or alternative communication methods (Makaton / PECs).
- Safeguarding Participants: Identify and report concerns, working with stakeholders and agencies to ensure safety and well-being.
**You will ensure that those we support have an accurate and detailed person centered support plan that is delivered each day by**:
- Ensuring upport plans are accurate and detailed by completing daily checks.
- Auditing information to ensure it is logged accurately, and discrepancies are reported immediately.
- Ensuring staff are aware of what is required when logging care in accordance with software and procedures in place.
- Supporting and mentoring Care Support Workers to deliver activities, empowering them to contribute to personalised care in daily routines.
- Working alongside the Hub Leader, Senior Support Workers, and Care Support Workers to ensure a collaborative approach to care.
**You will provide person-centered care and support that enables each individual to embrace life and unlock their potential by**:
- Providing opportunities for the people we support to make choices and decisions.
- Using total communication techniques tailored to each individual. Supporting people to speak up for themselves, and/or advocate on their behalf.
- Knowing the person, their care plans and contributing to their care planning via our care planning platform.
- Supporting with health, well-being and personal care, including continence and behavioral needs, as required.
- Independently follow written plans and guidelines for each person.
- Supporting diversity and respecting people’s age, gender, sexuality, ethnic origin, religious/cultural background, abilities/disabilities, and other needs.
- Encourage and empower the people we support to take part in all aspects of the day at the Day Service.
- Listening to individuals and acting on what is important to them.
- To use manual handling techniques to support individuals to access our fleet, be safely secured whilst traveling and to mobilise around the building and the in the community.
**Essential Skills**:
- Experience in a caring environment with complex needs.
- Kind, caring, empathetic and passionate about supporting people to achieve their potential.
- Team player with a supportive attitude.
- Can-do approach to support.
- Physically able to assist with wheelchair use and manual handling
- Proficient in English for clear and accurate recording
- IT skills for eLearning and care records
- Creative in designing engaging activities for complex needs
- Knowledge of total communication techniques (e.g., Makaton, PECs)
- Strong adaptable communication skills
- Collaborative with care teams, therapists, and families
- Experience in leading activities
- Committed to promoting independence, dignity, and quality of life
- Experience with risk assessments (Desirable)
Additional responsibilities may be assigned by your line manager as required, in line with the evolving needs of the role and the organisation.
