**Job summary**:
We are seeking a motivated individual with strong leadership skills, who can demonstrate the ability to manage a team, ensure high-quality care, whilst driving service improvement. The team lead will be expected to lead the newly established Transitions Team and continue to successfully support young people to transition from our inpatient settings to the next stage in their recovery, alongside supporting the ongoing expansion of the transitions service.

This is an exciting opportunity to lead and develop a team to add value to young people's inpatient experiences supporting their recovery back into the community.

The successful applicant will have the opportunity to liaise with multiple agencies, support family networks and work alongside inpatient and community services adding additional support to the service that is being provided to ensure that discharge from hospital is supportive and sustainable.

This role provides the opportunity to be creative and flexible in your approach, supporting a solution focused and social prescribing ethos.

The Team Leader will have the opportunity to develop their team to meet the needs of the young people and their families. They will have the opportunity to be creative with bespoke packages of care that are supported by the network and run a service to be proud of.

**Main duties, tasks & skills required**:
To liaise with relevant agencies in the delivery of care to young people and their families.
To provide time limited evidence based therapeutic interventions and to provide support to young people and their families/carers following discharge from inpatient care.
To promote and facilitate the participation of service users and carers in their own care and in the planning and development of services.
To take clinical lead and full line management responsibility for transition service ensuring they deliver a high quality, young person focused service.
To lead and manage referrals into the Transition Service.
To lead on and undertake outreach and group work to support young people and families.
To work with colleagues within Child and Adolescent Mental Health Services to ensure delivery of comprehensive after care from inpatient admission

**Qualifications**:
**Essential**:
Registered Nurse/ Social Worker/ Occupational Therapist with previous post registration experience

Current professional registration

Qualification in leadership and management, to masters level or equivalent experience.

Further evidence of professional development and training.

**Experience**:
**Essential**:
Demonstrable post registration experience of working with Acute Mentally ill and/or Seriously Mentally ill young people, their families and carers.
Experience of managing a team.
Experience In facilitation of groups.
Specialist training in self-harm.
Working knowledge of ICPA and risk assessment and management plans.
Ability to assess, plan, implement and evaluate complex treatment programmes.
Understanding of medication management issues and psychotropic medication.
Working knowledge of safeguarding for young people.
