The Wokingham Community Older Adults Mental Health Teamis looking for an enthusiastic, caring and highly motivatedMental Health Practitioner(Social Worker, Mental Health Nurse or Occupational Therapist)to join our friendly and innovativeHome Treatment Team.
Your base would be Wokingham Community Hospital where we have access tofree parkingfacilities,staff room and a friendly and welcoming team. There is also the opportunity to work remotely, and a balance of office and home would be encouraged.
The Home Treatment Team is a multi-disciplinary team with specialist skills in caring for older people with mental health needs in the community. The post holder will take responsibility for the assessment and implementation of intensive care and treatment in the homes of people with functional mental health problems, as well as people with dementia.
About the Job
- You will be monitoring the mental health needs of patients and working alongside their carers and assessing care needs under the Care Act where appropriate. Knowledge of appropriate legislation such as the Care Act, Mental Capacity Act and Mental Health Act is key to this post.
- To identify and select from a range of health, social and voluntary agencies which could assist and improve the care of individuals and their families.
- To monitor and evaluate the agreed care plan, making adjustments as appropriate in collaboration with the wider team.
- To provide specialist consultation and advice on issues relating to the care of older people with mental health problems to other professionals within the wider care community such as Residential Homes, Sheltered Housing, Day Centres etc.
- To have knowledge of therapeutic interventions such as crisis intervention and problem solving

You will be an experienced practitioner (Social Worker, Mental Health Nurse or Occupational Therapist) with developed clinical skills and experience
Be competent and have a pro-active decision-making approach to managing complex needs
You must be enthusiastic, caring and highly motivatedwith a passion and understanding for working with Older People
Be a car driver with the ability and readiness to travel
