We have a fabulous job opportunity for you to join our awesome team as our Support Coordinator. We support our customers who have mental ill health to move forward in their lives. Our service provides support to 26 adults living in their own self-contained flats within our Buller Court complex. 3 of our units are specifically for those with Autism.

You may be a Support Worker ready for your next step up or you may be looking to broaden your experience in a different setting. Either way, as a Support Coordinator you’ll be working in a person-centred way to manage your caseload and empower our customers to achieve their Aspirations. You’ll do this by working on a one to one and small group basis with customers, partner agencies and family members involved in their support.

You’ll use our brilliant suite of LIFE (Living Independently, Feeling Enabled) tools. You’ll be key in helping to shape our service delivery to be person centred and psychologically informed, supporting customers to achieve their aspirations. As well as your Line Manager you’ll have the support of a Mental Health Practice Lead based at the service for expert guidance and advice.

**About you**

As our Mental Health Support Coordinator, you’re passionate and motivated to help vulnerable people make positive and lasting changes to their lives. You’ll impress us with your ability to work on your own initiative whilst also being a natural collaborator You’ll have some experience in safeguarding, completing risk assessments and support plans.

It would be amazing if you’ve had a job in this type of environment before, however it’s your “can do” positive attitude and enthusiasm that are critical to make this role a success. We can nurture your talent, just let us know in your cover letter why you believe you can do this role.

To make sure that we keep our records straight, and customers safe, it's really important that you are technologically savvy (or at the very least not a technophobe). Training will be provided for all staff as part of their Induction process. Here at Home Group, we have invested heavily in our future and use current and up to date equipment. You’ll be issued with the latest kit, which could include a Surface Pro and mobile.
- When you join as our Support Coordinator, you’ll need to have a valid Enhanced Check under the Disclosure and Barring Service. The great news is that we will pay for that to be done if you don’t have a transferrable one._

**Working hours**

Buller Court is a 24/7 service so we’ll need you to be there for our customers. We see flexibility as a two-way street to ensure you have the time you need to do the things that matter to you with the people that matter to you

You’ll work on a rota basis and take your turn working days, evenings, sleep-in shifts, weekends and bank holidays to ensure our customers can access support at all times.

This is an exciting time for us as the service opened during lockdown and we are now enjoying being able to set up lots of exciting new initiatives post Covid, particularly with our brand-new wellbeing hub
