**Hours**: 30 hours per week
**Salary**:£18,260 (FTE £22,825)
**Reporting To**: Contract Manager
**Key Relationships**: Stop Smoking Practitioners, Hospital Ward Staff, pharmacy, community pharmacies, Tobacco Dependency colleagues (Ipswich and Colchester)

**The Role**:
To provide the delivery of an evidence based behavioural change smoking cessation programme and tobacco treatment for tobacco users ages 12 years and over in acute in-patient care[CM2].
**Responsibilities**:
- Be responsible for facilitating 1:1 Stop Smoking sessions to individuals whilst an in-patient at Ipswich hospital, to enable them to quit smoking in an effective and sustainable way as part of a 6 day a week provision. This post will require some weekend working (Saturday).
- Provide telephone interventions for clients e.g. pre op referrals and those upon discharge.
- Use coaching strategies, including goal setting and behaviour therapy skills to negotiate change to overcoming barriers to achieve smoking cessation.
- Exchange factual information with patients using persuasion, reassurance, tact, empathy; overcoming barriers to understanding, e.g. patient/client has physical impairment, mental health condition or learning disabilities.
- Support and assist other staff members to deliver care as delegated and directed.
- Support and encourage adults to set realistic goals in terms of changing behaviour and achieving positive healthy outcomes.
- Be responsible and accountable for clinical administration tasks and management of resources within the clinical area that are deemed necessary to the role. e.g. specific clinical tasks to support the smooth running of the service, such as NRT/Medication stock control within ward(s).
- Be sensitive and supportive to the psychological and physiological needs of participants, and signpost additional support where appropriate.
- Regularly evaluate the success of the programmes through self-data analysis and provide feedback to ensure continuous improvement in terms of outcomes.
- Adhere to local and national safeguarding procedures.
- Support the patient assessment and monitoring process.
- Undertake timely and accurate computerised and paper collection of data on patient referrals, contacts and outcomes. Ensuring all data is updated within 2 working days.
- Ensure all recording systems are up-to-date and provide reports, interpreting and analysing the information in a clear and concise manner.
- Support engagement and networking to promote the service to Stakeholders, clients and health professionals in a range of different ways. Online meetings, video calls and telephone calls where necessary.
- To promote the welfare and safeguarding of our clients, families and children and to support Morelife in safeguarding children and vulnerable adults.
- Recognising signs or harm or potential harm or abuse and escalating the matter to the safeguarding lead without delay.
**General**:
- To positively promote access to the community stop smoking services through onward referral from hospital discharge.
- To organise and engage with ward staff, various hospital departments and NHS colleagues as necessary to enhance engagement and to develop and facilitate networks and opportunities for referral into the programme.
- To ensure that a professional service and image is maintained at all times, thereby acting as a role model to all staff.
- To support a leadership style which is underpinned by strongly held values around equality, diversity and openness; effectively builds and maintains relationships with team members.
- To record, monitor and improve on KPIs.
- Monitor own activity against targets, engagement work, campaigns and events, as well as referral numbers to ensure programmes and activities meet necessary needs of stakeholders, commissioners and demand.
