Housing Solutions Officer - Hospital discharge and health services

This is a fantastic opportunity to join the busy, exciting Housing Advice and Options Service. Housing Solutions officers provide housing advice, assess households in housing need, and provide the best available support and housing options tailored to each households specific needs. A solid customer first approach is essential to the role. Homeless prevention is at the heart of everything we do, and if this is not possible, we try to find the most suitable alternative accommodation solution.

We have one full time role (37hrs) Fixed Term for 12 Months.

You would be based flexibly across Sheffield at multiple sites, including but not restricted to Howden House, Northern General Hospital and Royal Hallamshire Hospital mainly at the Northern General Hospital.

You must have excellent organisational and communication skills with the ability to document and record notes on IT systems. A significant part of the role will be liaising with patients and their relatives to ensure a seamless, timely discharge.

The applicant will be expected to be enthusiastic with the ability to use their own judgment to resolve issues. They should be able to work well under pressure and be able to prioritise their own workload under constantly changing demands.

A critical success factor in this role is reducing the length of stay in hospital and ensuring that the discharge is planned and executed for all patients.

Multi-agency cooperation and partnership is fundamental to this role, and will entail working closely with partner agencies, primarily but not restricted to Adult Social Care as well as General and Mental Health Hospital services across Sheffield to help facilitate discharge for clients who are homeless and in hospital, and to ultimately prevent and relieve bed blocking. This role will entail visiting service users in person or virtually, and working intricately alongside partner agencies to assess circumstances and investigate cases, and to gain a thorough understanding of service users and the holistic needs of their household.

You would be based flexibly across Sheffield at multiple sites, including but not restricted to Howden House, Northern General Hospital and Royal Hallamshire Hospital and would be working with multiple teams with skilled and experienced officers, seniors and managers who would be on hand to support you.

We need excellent communicators who work well on their own initiative but are also flexible team players. It is important to be resilient and hardworking, with a keenness to learn. Flexibility and adaptability are essential to this role, to allow quick responses to discharges, referrals or enquiries from partner agencies and clients both in person or virtually.

If you are appointed to this role, your starting salary will be at the bottom of the grade. If you provide payslip evidence that your basic pay is greater than the bottom of the grade, we will consider starting you at the point of minimum advantage within the grade.
