**Day Services Lead**

**job Type**:Full Time, Permanent 37.5 Hours a Week

**Salary**:£38,500 Per Annum

**Location**:Wrexham

Our client provides specialist palliative care services, completely free-of-charge, to patients and their families across a wide area stretching from Wrexham, Flintshire, and East Denbighshire to Barmouth and the border towns including Oswestry and Whitchurch.

**The Role**

**Responsibilities**
- To coordinate all outpatients’ services including day care services, and outreach projects.
- To provide specialist physical, psychosocial, and spiritual care to patients attending the Day Care Services and Outpatient Services.
- To line manage the day services, nursing team, session therapists, including Art, Music, and Complementary Services.
- To coordinate input from administrative support staff into day services/out-patient services.
- To ensure adequate and appropriate staff cover for outpatient clinics, and services
- To participate actively at MDT meetings and clinical decision-making.

**Professional Responsibilities**
- To act as a resource for the team, to empower and lead the team.
- Ensuring delivery of high-quality care and accurate assessment of the needs of the patients and their families, planning, implementing and evaluating care. Work jointly with all MDT members.
- To act as a role model, demonstrating effective leadership and management of the team.
- To ensure all patient documentation is accurate and that data is safely stored adhering to GDPR regulations.
- To liaise appropriately with primary and secondary care teams, social services, and other care agencies, ensuring continuity of patient care and promoting good communications and partnership working.
- To develop, implement and evaluate individual and group work with patients and their families in partnership with members of the rehabilitation team, and other teams and therapist within NHH.
- To undertake holistic patient and carer assessments for day services, and outpatients or domiciliary visits if appropriate.
- To promote high quality palliative care in the community through professional networking and participate in specialist training programmes as required.
- To co-ordinate the multi-Disciplinary care of patients attending Day Services. Drop Ins and Outpatient Clinics.
- Demonstrate an understanding for service demand, ensuring patients and families are seen at the 'right time in the right place by the right people’.
