Older Person's Mental Health Practitioner

We are the South Somerset OP-CMHT and owing to a current member of staff moving forwards with a career change we have a vacancy within the team. We are looking for a motivated, caring, forward thinking individual who is either aNurse, Social Worker or Occupational Therapistby profession.
The post holder will deliver a high quality, comprehensive mental health service for older adults of with severe and enduring mental health difficulties both in Older Persons Mental Health Specialist Care Settings, the Community and those in Memory Assessment Service. The role involves managing a defined caseload with a focus on working with individuals and their significant others who experience a complex range of needs and difficulties. Acting as a key member of the multi-disciplinary Team, the post holder will provide assessment, psychosocial, evidence based interventions using a collaborative, recovery approach and will be skilled at positive risk management. Liaison with a wide range of agencies is central to the role.
The team provides needs-led assessment and treatment and support, for older-aged adults in the community, working closely with the Inpatient Ward, Home Treatment Team, IDSS other CMHTs, GPs, Primary Care Network MDTs and Voluntary Services, and in the developing neighbourhood integrated teams.
You will make ongoing assessments of service users’ needs to reflect individual circumstances, including; assessing risk, promoting and enabling independent functioning, delivering relevant therapies and ensuring that service users are protected from significant harm.
You will be committed to a person-centred approach to your work and will have an ability to prioritise and work flexibly. You will hold a caseload that will include patients with functional and organic diagnoses. You will also be part of the Duty worker system that is in place.
The team's hub is based inYeovil at Magnolia House, the spoke site isBracken House in Chard. You will be expected to work across both areas to cover the South Somerset locality.
You will help to supervise the work of other registered, unregistered and vocationally qualified staff under the direction of the team manager.

Liaising and working in partnership with acute inpatient services, in-patient and community medical staff, pharmacy practitioners, psychological therapists. A range of community mental health Teams within and outside the organisation, primary, out of hours GP service, Emergency Duty Team, Community Directorate, housing providers, voluntary agencies, Adult Social care, PCT, substance misuse services, the police other emergency services, the Probation Service, NHS Direct and the Safeguarding Children Team.
