**About the Role**

We are currently seeking a dedicated Health and Employment Coach to join our team. As a Health and Employment Coach, your primary role will be to provide essential support to participants in effectively managing their health conditions and promoting overall wellbeing, with the goal of helping them move closer to employment.

Your responsibilities will include creating personalised Health and Wellbeing plans for participants dealing with both mental and physical health issues. This will involve establishing strong partnerships and referral pathways with local health organisations, as well as serving as an advocate for our participants. Additionally, you will actively engage with referral organisations to develop effective pathways for participants in need of specialised support, ensuring they are connected to the appropriate resources. As part of your role, you will also actively seek to understand the local health and wellbeing landscape in Greater Manchester, building relationships and partnerships with local organisations and explaining our program to various stakeholders.

Collaboration with external agencies will be essential, as you work together to identify suitable health and wellbeing interventions that complement participants' employment action plans.

**About You**

You will have exceptional interpersonal skills and feel completely at ease conducting short training and advice sessions, covering topics such as mindfulness, healthy eating, and disclosing health conditions. Flexibility is key, as you will need to adapt to remote delivery methods when necessary, requiring a solid foundation in IT skills.

A genuine passion for supporting individuals in managing their health conditions and promoting overall wellbeing is a fundamental requirement. With your strong ability to develop comprehensive health and wellbeing plans, you will guide participants towards achieving their desired outcomes. Building a network and establishing partnerships with local health organisations is essential, as you navigate the local health and wellbeing landscape and create referral pathways.

Excellent communication and advocacy skills are vital for effectively representing the interests of participants, while maintaining a proactive approach to understanding the needs and nuances of the local community.

A minimum level 5 health and social care/clinical qualification or equivalent, coupled with extensive experience in a relevant health, care, or wellbeing setting, is expected. An additional advantage would be holding a minimum level 3 Employment Related Services/Information, Advice, and Guidance qualification or equivalent experience.

**Salary**: £28,000.00-£34,000.00 per year
