Looking after individuals in a care setting, especially those with Dementia, can be challenging at times, and no two days are the same. **Role Overview**:
The Care Co-Ordinator is responsible for caring for people with a wide range of needs as well as dispensing medication and keeping care plans up to date. This will be a leadership role assisting the Deputy and Manager by overseeing the Senior team, taking responsibility for effective handovers, supervisions and inductions for Senior Care Assistants and assisting with basic level management of the team.
**Role Expectations**
Provide the highest level of personal care and attention to residents ensuring that all contact is polite, friendly, warm and supportive
Recognise deterioration of residents and seek appropriate support / advice / referral
Provide and promote a high level of end of life care be able to develop end of life care plans show empathy and compassion for relatives
Participate in developing high quality person centred care plans, incorporating individual risk assessments for each resident, plan and proactively review care plan evaluations with residents and families
Administer medication to residents as prescribed, accurately maintaining appropriate records, administer controlled medication in line with current guidance, order and return medication, check in and change over medication on new cycles following current guidance and the home policies and procedures
Assist with Medication change overs
Check fluid intakes and action any concerns
Carry out audits as requested by the manager
Attend regular meetings with members of the management team to provide weekly updates of residents ( care over view meeting )
Carry out Senior team supervisions.
Hold the responsibility of first reporting line for the Senior Team.
Assist with and oversee Senior Team inductions and chase training compliance.
Completing spot checks.
Evidence Quality daily walk rounds - given to deputy weekly
Ensure meaningful hand overs are carried out
Provide and promote positive leadership within the care team
Ensure that all staff contribute to the efficient running of the service and supervise staff in practice who you support / including inductions process for new starters - long side deputy manager and admin support
Ensure the health and wellbeing of all staff working within the home
Develop effective working relationships with other employees
Work in cooperation with members of the multi-disciplinary team, take part and when required arrange multi-disciplinary team meetings
To liaise with other care professionals relating to the care needs of the residents
Work with the management team to ensure the vison and focus for the home is promoted and communicated to the care team
Maintain high levels of infection control and work within current COVID 19 guidance which may be applicable at the time - understand reporting guidance regarding any infection status
Maintain skills at a current level, and undertake any training relevant to role be willing to work towards a level 5 health and social qualification
Seek opportunities for personal and professional growth
**Working Hours**:
Core hours 8am - 5pm 5 days over 7 days a week. Hours may differ in line with business need. On call responsiblity
