**Service Support Worker - Assisted Discharge Service**

**Location: Hove**

**Contract type: Permanent**

**Hours per week: 31.5 hrs per week**

**Salary: £18,379 per annum pro rata (depending on experience)**

As a Support Worker, you will provide the support and care which can make a vital difference to the quality of people’s lives, transporting them home in Red Cross vehicles and allowing them to continue living safely at home.

You will provide reactive, focused and flexible support to adults following a hospital stay to enable early discharge when clinical needs have been met. You will assist the Independent Living Service Coordinators by undertaking duties required to meet the service users’ needs in order to re-enable service users to remain in their own home and prevent hospital re-admission.

You will be providing practical and emotional support in the service user’s own home and signposting to other services where appropriate to enable the individual to live independently in their own home.

The Independent Living (IL) & Crisis Response (CR) service (IL/CR) supports people in the space between hospital and home and responds to the needs of people in crisis following an emergency, maintaining an extensive network of external relationships across health, social care and emergency services.

- You’re professional. You can deal with queries in a diplomatic and confidential manner
- You love being flexible. Working hours out with the norm suits you.

**Also, you should be the following**:

- Educated to GCSE level (or equivalent by experience)
- IT literate
- Full driving licence holder for at least 3 years and aged 21 yrs. (manual licence essential)
- Have a good knowledge of services provided by the NHS and Social Care

Please note due to the context and duties of this role, in line with government guidelines it is not suitable for individuals who are clinically extremely vulnerable or clinical vulnerable to undertake this role.
