The post holder will work within an established, integrated, multi-disciplinary team, focusing upon individuals with severe and enduring mental health problems. The post holder will undertake responsibility for the care co-ordination and case management of a defined caseload of clients with complex and challenging needs as defined within Parts 2 and 3 of The Mental Health(Wales) Measure 2010. Within their role as Care Co-ordinator the post holder will facilitate the promotion and maintain a high standard of nursing care adhering to current legislation, professional and corporate guidelines.
