Are you an experienced Mental Health Practitioner looking to work autonomously, in a forward thinking, fast-paced environment?

Whether you are a Registered Nurse, Occupational Therapist or Social Worker; this is the ideal challenge for someone who is looking to make a difference to the local community of North Norfolk.

You’ll be working alongside an experienced lead mental health practitioner, as well as alongside Mental Health Practitioners based at the different surgeries. We’re a collaborative team and work closely with all those who are based at the practices.

Our Primary Care Network (PCN) Service for Mental Health has been developed to provide access to mental health services in Primary Care as part of an integrated health and care system, which brings care closer to home for our population.

The Team are passionate about developing their mental health provision; they’re proactive and enthusiastic and need a forward-thinking, driven mental health practitioner to help them achieve their aspirations.

As a PCN MH practitioner, you will work across the interface between Primary Care and Secondary Mental Health Services undertaking the planning and delivery of direct patient care.

We’re committed to training, continuous professional development and ensuring all members of the team can access resources to help them to develop and progress. When you join us, you can expect to be supported from the start; you’ll spend time with our community teams learning about the different roles they have, as well as shadowing other members of the PCN.

These roles, although new have been piloted in our Early Adopter sites for the past year so the workload is defined, manageable and appropriate to allow for patient centred care and other activities
