Care Coordinator

We are looking for a Care Coordinator to join our team and effectively meet the needs of all customer care packaging. You will provide a good quality service to its users, enabling our customers to live independently with choice and control in their own home.

**Care Coordinator Duties**
2.1 To coordinate the services rota system to schedule care on a daily/weekly basis, ensuring that all customer calls are covered and that Independent Lives care service team and the Home First team of care support workers are allocated work
2.2 Review and monitor the rota and make adjustments to accommodate the needs of the customer upon request
2.3 Make necessary arrangements in the case of an emergency call from a customer
2.4 Ensure that the rota is kept up to date and that all information is accurate
2.5 Work with the HR team to inform on staffing requirements and areas that need development
2.6 Manage QAs effectively, ensuring they reflect the accurate hours delivered.
2 Schedule spot checks and supervisions, ensuring quality compliance
2.8 Identify growth opportunities and actively seeking new customers
2.9 Support with sickness absence management
2.1 Maintain accurate records of annual leave/other absence on scheduling software
**On-call/Out of Hours Duties**
To help coordinate and be part of the on-call/out of hours rota if required.
**Administration**
Maintain regular contact with care support workers and customers to keep everyone informed of all rota changes.
4.2 Ensure that all customer care plans are up to date on the PASS and contain accurate customer information.
4.3 Ensure that customer information is stored appropriately and confidentially.
Maintain and update other service documentation as required.
4.6 Support the head of care and in the maintenance of the CQC registration.
4.7 Ensure that all external communications are performed in a kind and friendly manner
