An exciting opportunity has arisen in the Northumberland Children and Young Peoples Service. We are recruiting band 6 Community Practitioner to work into our Mental Health/Learning Disability Pathways
Our service is fully integrated and provides specialist mental health assessment and intervention for children and young people with complex and enduring mental health problems, including those with a learning disability. The multi-disciplinary team comprises of a highly skilled and experienced workforce.
To be responsible for the assessment and management of casework of service users who have substantial and complex mental health needs.
To provide evidence based interventions following a full assessment as part of a multi-disciplinary team. To complete documentation within robust timescales, and ensure review.
To undertake duties in supporting families and partner agencies, and attend multi-agency meetings to safeguard children.
Northumberland work from 5 bases, and your base will be allocated to the closest to your home address (Bases in Alnwick, Hexham, Blyth and Morpeth).
We deliver a wide range of evidence based interventions through clearly defined care pathways. We have developed a comprehensive supervision structure for staff including individual supervision and peer support, through our care pathways. We offer many interventions to young people and their families, and offer training via our internal training programme.

The service operates within the North business unit of CNTW and works across the geographical area of Northumberland.
The team functions as an MDT where we work collaboratively with nurses, support workers, occupational therapists, social workers, psychology and psychiatry.
