Job Summary Universal Crisis Team Newcastle and Gateshead have a vacancy for a Full Time Community Practitioner for the Working Age Adults pathway. We are looking to recruit motivated and committed individuals into the teamp> Responsibilities To assess and manage casework of service users with substantial and complex needs. Provide specialist clinical advice to service users/carers, staff, statutory agencies, third sector partner agencies and the public. Collaborate with service users/carers to develop recovery and wellbeing focused care plans, ensuring regular reviews to monitor progress and adapt plans as necessary. Consider carer needs and involve carers/family in care planning where appropriate. Deliver evidence based clinical and psychologically based interventions. Coordinate treatment planning & interventions, reviews and discharge planning. Provide compassionate care based on empathy, kindness, respect and dignity. Incorporate a client centred philosophy into nursing practice and client centred collaborative care. Job Details Band: 6 Salary: £38,682 to £46,580 per annum Contract: Permanent Working pattern: Full‑time Reference number: 263-CCG25-380-HN Location: St Nicholas Hospital, Jubilee Road, Gosforth, NE3 3XT Closing date: 9 December 2025 Person Specification Education and Qualification (Essential) Professional qualification and current professional registration as a first level registered nurse (RNMH, RMN, RNLD) or equivalent as an AHP, social worker or clinical psychologist. Evidence of post‑registration training in clinical/psychological interventions/skills relevant to the post. Proven track record of post‑registration clinical experience. Mentor training. Training in CPAP (Care Programme Approach), safeguarding, MAPPA (Multi‑Agency Panel for the Protection of Vulnerable Adults) and child protection. Up‑to‑date knowledge/experience of relevant legislation. Desirable (Essential) Post‑registration qualifications in psychological interventions. Experience of working within a community team. Non‑medical prescribing. Knowledge and Experience (Essential) Awareness and understanding of professional accountability, NMC standards and other local and national initiatives. 12 to 24 month band 5 qualified nurse experience dependent upon service need. Skills and Competencies (Essential) Proven ability to complete comprehensive assessment of complex mental health needs. Risk assessment skill & management. Identify treatment options as per NICE Guidelines. High standard of written and verbal communication. Ability to work under pressure.
