We are currently looking for a Senior Practitioner Social Worker to join the Integrated Discharge Team at University Hospitals Derby and Burton.
The team primarily focus on transfers of care from Acute Hospital from Royal Derby Hospital and Queens Hospital Burton
The purpose of the role will be to lead on professional practice within the hospital discharge service, providing professional supervision and support to team members. You will shape best practice standards, support and encourage students and newly qualified staff and deputise for the Service Manager when required. As a Senior Practitioner, you will have the ability to motivate and support colleagues to achieve and exceed personal and team goals using a coaching and mentoring style.
The Senior Practitioner will develop and maintain professional relationships both within and across organisations and support the development of our local offer to Derbyshire residents. You will think innovatively throughout your practice and have creative approaches to avoid admissions and support discharges from hospital. You will demonstrate a strong practice model which uses a strength-based, home first approach to Social Work, supporting people to maintain independence, choice and control over their own lives.
The job may require unsocial hours working including weekends and Bank Holidays. Payment arrangements will be detailed in any contract offer made.
Saturday and Sunday hours will be paid at plain time where it is part of your normal working pattern unless it is between 7pm and 7am, where enhanced rates of pay apply. Enhanced pay rates are paid for Bank Holiday working.
