Patient Safety Learning and Investigation Lead

To co-ordinate, participate and lead formal reviews of patient safety related incidents both internally and externally. Implementing Trust wide methodology to identify lessons learned, action plan generation and support a Lincolnshire system approach to the management of patient safety incidents.
To lead on implementation of the Trust patient safety incident response plan (PSIRP) as part of the National Patient Safety Strategy and Patient Safety Incident Response Framework (PSIRF).
To lead PSIRF learning reviews with patient and family involvement for patient safety related innovation, improvement and learning to enhance the quality and safety of LCHS compassionate care delivery
To ensure that recommendations from investigations and reviews will lead to organisational learning and quality improvement providing feedback reports and leading learning events to share and disseminate learning. Contribute to the development of an organisational and system learning approach

LCHS recognises the increasing demand for more flexibility in working practices in order to accommodate the personal goals and commitments that employees experience at different stages during their working lives. Flexible working opportunities such as part-time working, compressed hours, job shares, and hybrid working models can help staff to balance their personal and work commitments. Arrangements can be put in place if it can be shown that the needs of the individual can be balanced with those of the service and the impact on other team members, enabling the Trust to recruit and retain skilled staff, raise staff morale, reduce absenteeism and respond to changing service needs more effectively.
To lead on a range of review methodologies as outlined in the patient safety incident review framework and wider patient safety investigations to conduct reviews to support learning and improvement. To undertake thematic reviews in association with clinical colleagues.
To triangulate incident report analysis alongside other intelligence for themes and trends within the Trust, sharing triangulation findings for wider generation of lessons learned, action planning which will contribute to weekly, monthly, and incident report writing.
Construct and deliver investigation training to support staffs evolvement and utilisation of the acquired skills and knowledge, supporting their participation within patient safety investigations with oversight from this role.
