The post holder will be highly motivated and experienced therapy/rehabilitation assistant who will participate in the care, rehabilitation and reintegration of patients with spinal cord injury.

This role is being funded by the NHSE South East SCI Transformation program and the post holder will be supporting a 12-month Pathway Improvement pilot. This individual will support patients within the discharge process and assist their transition back into the community, acting as a key contact for the patient and any relevant care providers.

Elements specific to the pilot are;
- Accompany patients with a complex discharge requirements home upon discharge
- Be a point of contact for the patient up to 6 weeks post discharge
- Support all discharged patients (Adults) to complete the Craig Handicap Assessment Reporting Technique (CHART) within 6 weeks post discharge. This will be done via a virtual appointment. CHART is designed to assess how people with disability function as active members of their communities.

Promote and encourage rehabilitation activities for spinal cord injury patients through therapy, education and leisure activities. Under the direction of Multi-disciplinary Team Members (MDT), assist the patient to carry-over rehabilitation techniques from therapy/ward, to the home/community environment to assist in maximising patient recovery and progress.
- To work independently with patients, under the guidance of trained staff, on a ward, in a therapy environment or within a community or home setting. To report back to trained nursing and therapy staff as appropriate.
- Provide practical education regarding spinal cord injury and individual care needs to patients and families.
- Actively work with spinal cord injury patients with a variety of dependency levels including ventilator dependent patients, participating in their rehabilitation reintegration and education programmes.
- Encourage a co-operative and collaborative working relationship with the multi-disciplinary team and attend multi-disciplinary meetings as appropriate.
- Due to the nature of the post, flexible working practices are required in order to facilitate leisure and reintegration activities, etc.
- Demonstrate the ability to adjust communication and therapeutic approaches according to the patients needs.
- To work with the patient and MDT members to plan patient specific rehabilitation activities

Clinical
- To provide care and assistance to SCI patients in a variety of settings.
- Promote and encourage social reintegration for patients under the care of the Spinal Cord Injury Centre team.
- Liaise with SCIC MDT regarding social reintegration and hospital/community-based rehabilitation. Assist patients to participate in appropriate activities in order to enhance the rehabilitation process.
- Accompany patients with complex discharge needs home upon discharge.
- Liaise with relevant 3rdparties in relation to discharge plan i.e Community Occupational therapist/physiotherapist, social worker, care agencies.
- To uphold a professional relationship with patients, and their family members.
- To attend and participate in patient meetings and other meetings as required, including MDT meetings, Ward Rounds and Goal Planning.
- Assist in empowering patients to manage their finances and general correspondence from a variety of agencies.
- To work with MDT members to escort patients on home visits and other community meetings providing appropriate care as needed.
- Act as a driver for patients during social reintegration activities, using the NSIC vehicle or alternative appropriate roadworthy vehicles.
- To provide support and assistance to patients who are deemed suitable to stay in the onsite bungalow. Enabling these patients, the opportunity to experience independent living before they are discharged.
