**Job Title**: Care Coordinator
**Responsible To**: Registered Manager
**Hours of work**: As per contract
**Purpose of the role**:To lead care assistants in the delivery of a caring compassionate care to the elderly and vulnerable people in the local community
Working closely and as part of the health care assistant team you will ensure that clients are well taken care of in their private residents in a person-centred way.
**Key Responsibilities**:
- To organise the care team whilst considering the changing needs of clients.
- To clearly set out the expectations for each shift and direct the team accordingly.
- To promote dignity, respect and choice for clients; ensuring that the care team work
within this ethos at all times.
- To be accountable to the Registered Manager at all times; ensuring that he is kept informed of any concerns or changes to the health and wellbeing of individual client.
- Ensure that care and support is provided for clients in a timely way; upholding and
improving standards of care delivery.
- To ensure that clients have daily opportunities for meaningful occupation and activity.
- To ensure that all care staff engage in the activity and occupation program of the agency;
supporting clints’ involvement to enhance wellbeing and quality of life.
- To administer medication, should this be required due to the requirements of the client.
- Uphold and embed the company values and ethos at all times
- To act as a role model and be an exemplar to the care team at all times
- To have a good understanding of individual client’s needs and be able to respond
effectively and responsively to changing needs.
- To help drive improvements in care delivery by upskilling the care team members, through observation and mentoring.
- Support and mentor new team members.
- To ensure the emotional and wellbeing of clients at all times.
- Team work - covering team leader shifts when required with a flexible working pattern.
- To promote good communication among the team members
- To assist the Registered Manager through delegation to complete designated responsibilities such as care plan reviews, recruitment, disciplinary procedures, responding to staff and clients’ complaints etc.
- To contribute to the care planning process for individual client by sharing information with the Registered manager about their needs, wishes and any changes that you have noted.
- To ensure that all compliance records are accurately completed and ready for inspection by the regulator.
- To ensure that all safeguarding processes are always followed.
- To monitor the care practice of individual carer’s, addressing any issues of concern
proactively, including reporting and documenting as required statutory responsibilities
- To ensure that all staff take reasonable care of their own and other people’s health and safety whilst at work
- To ensure that all staff attend and follow training provided by the organization as to maintain good care standards and are familiar with relevant policies and procedures.
- To ensure all staff report accidents, incidents and unsafe practices to the appropriate person.

**Scope of Job Description**

**Qualification**:
Level 6 diploma in SVQ Health and Social Care desirable but not essential because free training will be offered. However, the person must have worked in a domiciliary care setting as an healthcare assistant for a period not less than 2 years.
Must be willing to train for level 7 SVQ diploma in health and social care after successful completion of Level 6 SVQ diploma if not already achieved
**Salary**:
£22,000 to £25,000 pa depending on experience
**Job Types**: Full-time, Permanent, Zero hours contract
Pay: £22,000.00-£25,000.00 per year
