St LeonardsonSea, United Kingdom East Sussex Healthcare NHS Trust Full time

Join us in this all new and very exciting venture, blending medical innovation with bespoke care, bridging the gap between Hospital and home. We believe in delivering safe, effective and personalised healthcare at home, supported by a wider team of dedicated professionals.

ESHT are collaborating to bring acute care to people in the comfort of their own homes. We know that people recover faster and better in their own environments and Virtual Wards endeavours to support this.

Our Virtual Wards programme aspires to help people to stay well and at home by facilitating self and supported monitoring of:

- Chronic conditions in the community, aiming to reduce the need for admission to hospital
- Acute illness, aiming to ensure as little time as possible is spent in an acute hospital bed

We are currently recruiting for a Specialist Dietitian to support across our Virtual Wards and Community Nutrition Support teams here at ESHT.

You will be providing comprehensive dietetic assessment, treatment and management as part of an integrated multi-disciplinary team. Consultations will be supported using a blended approach of both in person and remote monitoring of our patients across the ESHT area. There will be further opportunity to develop your skills as you rotate between our Virtual Ward programme and Community Nutrition Support team

We will support you with full training to help you make a success of your new role you will also be part of a wider team of 40+ Dietitians covering all specialities to include CNS, Acute, Oncology, Diabetes & Prescribing Support. This truly is an exciting time to be joining ESHT.

We are proud to provide ‘Outstanding’ care and be a great place to work

We provide safe, compassionate and high quality care to half a million people living or visiting East Sussex. We are one of the largest organisations in East Sussex, the only integrated provider of acute and community care in Sussex. Our extensive services are provided by 7000 + members of staff working from acute hospitals in Hastings and Eastbourne, three community hospitals in Bexhill, Rye and Uckfield, over 100 community sites and in people’s own homes.

In 2020 the Care Quality Commission rated us as ‘Good’ overall, and ‘Outstanding’ for being caring and effective. The Conquest Hospital and our Community Services are rated ‘Outstanding’. Eastbourne DGH rated ‘Good’.

In 2020, the Trust launched its ambitious ‘Building for our Future’ programme. This once in a lifetime programme aims to repair, redevelop and expand our hospitals, transforming the environment in which we provide care for generations to come.
