**Your Responsibilities**

As a **Care Team Leader**, you will play a key role in ensuring the smooth running of care services. Your duties will include:
Conducting client **assessments and risk evaluations** for new clients
Regularly reviewing and updating **support plans** to meet clients' evolving needs
Carrying out **introductory visits** for new care workers assigned to clients
**Ensuring all client visits are covered**, including arranging replacements for absences
Taking part in the **on-call team** on a rota basis to handle emergencies
Collaborating with Care Coordinators for **staffing, care planning, and recruitment**
**Supervising, mentoring, and supporting** care workers in the field
Identifying **training needs** and supporting staff development
Monitoring **customer satisfaction** and maintaining high service standards
Ensuring **accurate record-keeping** and compliance with regulations

**What You Need to Succeed**

**Experience in a similar role** within Health & Social Care
**Level 3 Diploma in Health & Social Care** (preferred)
A **full, clean driver’s licence and access to a vehicle**
A **caring, reliable, and dedicated** approach to supporting others
The ability to **lead, motivate, and support a team**
**Flexibility** to work various shifts, including weekends and evenings

**Additional Details**

**Salary**: From £13.50-£14.00 Per hour
**Contract type**: Full-time, Permanent
**Shifts**: Day shifts, night shifts, Monday to Friday, and weekend availability
**DBS & References required** (SureCare can assist with this if needed)
