**Care Station NW is seeking a Care Coordinator** to join our established office in central Blackburn in a full-time permanent position. Would you like to be a pillar of your community offering support to service users within that community so that they receive the best possible care in their homes and the community. To succeed in this role, you will need to possess a calm and confident demeanour, ensuring a positive experience for our service users, and fellow team members. You will develop strong relationships across the business to ensure the smooth running of our personalised care services.
- Experience of working as a Care Co-Ordinator/Scheduler ( Essential ) or other senior care role such as deputy manager etc.
- 2 years’ experience in a care setting.
- Hold or be working towards level 4/5.
- Exceptional organisational skills are essential for this role.
- An analytical/data driven mind/way of working.
- Full Driving License and own car ,as this role will require travel.
**Key Responsibilities for the Care Coordinator role**:
As a Care Coordinator, you will play a pivotal role in ensuring the smooth and efficient delivery of our domiciliary care services. Your primary responsibilities will include:
- Creating and managing schedules for our team of staff to ensure clients receive timely and consistent care.
- Communicating with clients and their families to establish care requirements, preferences, and any changes in scheduling.
- Assigning and coordinating staff to clients based on their needs and preferences
- Maintaining accurate and up-to-date records of schedules, client information, and staff assignments.
- Addressing scheduling conflicts and adjustments promptly and effectively.
- Collaborating with staff, and healthcare professionals to ensure seamless care delivery.
- Develop and manage client relationships within the home care team
- Manage a team of homecare staff
- Complete monthly reports and care package schedules within deadlines
- Manage quality monitoring of staff / supervisions and appraisals
- Conduct quality assessments / risk assessments /monitoring of service users
- Participate in the on-call rota 1 in 3 weeks
- Adhere to the policies and procedures of Care Station NW
You must be approachable with excellent customer service skills and experience. you will be a clear communicator with excellent telephone manners and highly organised and self-motivated.
**Training & Development**:

- Attend mandatory training days/courses, on or off site, as and when required.

**Salary**: starts at £12.50 moving up to £13.17 on completion of 6 month probation
Pay: From £12.50 per hour
Expected hours: 37.5 per week
