**Social Care Assessor - Adults**
**Liverpool**
**£15 - £17 per hour**
The Talent Crowd are looking for a Temporary Social Care Assessor to work within a Adult Team for our client in Liverpool.
**Duties of the role**:

- To contribute to an initial assessment at both Contact and Overview Assessment levels as described within the Single Assessment Process under the guidance of the Team Manager / Social Worker.
- To contribute to the assessment process for equipment/minor adaptations as part of the Single Assessment Process under the guidance of appropriate professionals.
- To provide care plans and commission services following the completion of assessments under the direction of the Team Manager / Social Worker.
- To contribute to the initial reviews of assessments and make amendments to care plans/commissioned care plans as necessary, under the guidance of the Team Manager / Social Worker.
- To liaise with other partners e.g. Health, Housing as directed to ensure provision of services to meet identified needs.
- To undertake scheduled and unscheduled reviews of current service provision to individual clients, to check the effectiveness of the Care Plan in meeting identified needs, to check the quality and effectiveness of the provider and to review the needs ofthe service user, under the guidance of the Team Manager / Social Worker.
- To keep appropriate records in relation to the work undertaken by collecting and collating information required by Locality Team staff.
- To utilise ISSIS and other computer systems, inputting data In respect of the assessment and review work undertaken.
- To undertake duties in other Localities as identified by the Team Managers.
- To liaise as necessary with relevant staff in Contracts section about service provider issues.
- To attend and participate in meetings as required.

**Requirements**:

- Social Work Degree or equivalent
- Current registration with Social Work England
- Enhanced DBS
