The On Call Co-ordinator is responsible for coordinating the continuity of care and support outside of office hours within our supported living services. The role is to offer support, advice and act appropriately to calls received from the support teams. The role will also include rota management, ensuring appropriate cover is sourced to cover holiday, sickness and any other emerging situations within each service.

**Key Role Responsibilities**
- To receive incoming calls from support workers and seniors outside of office hours, and to make decisions in response to their call.
- To create and manage the rota, maintaining continuity of support where possible.
- To arrange cover for sickness and emergency leave.
- To escalate crisis issues to the second in line manager.
- To liaise with families of those we support regarding any out of hours issues.
- To contact the emergency duty team of the individuals we support regarding any emergencies that occur outside of office hours.
- To provide emergency support to staff and service users, and to respond in person if necessary.
- Provide on-call colleague with a detailed hand over on finishing shift.
- To record and report any changes or areas of concern.
- To advise Registered Manager of any unplanned expenditure.
- To liaise with all necessary parties involved in providing care to the individuals. E.g 111, Paramedics, Crisis team, Emergency Duty Services.
- To consistently promote a positive image of the company.
- To keep up to date and maintain a good general knowledge of Care standards and company policies and procedures to comply with all policies and procedures.
- To maintain good working relationships with all colleagues and individuals.
- To undertake any other related or special tasks applicable to the post as required by the Registered Manager.

**Essential Skills**
- Working knowledge and experience of the Health and Social care industry, regulations and legislation.
- Strong organisational skills with experience in managing staff rotas for Support Workers.
- Excellent written and verbal communications skills.
- Good planning and time management skills.
- Qualification in Health and Social Care.
- Effective IT skills including the use of Microsoft Word and Excel.

- Displays a confident and clear approach when advising support workers.
- The ability to build relationships with people quickly through effective communication.
- Displays an enthusiastic and positive attitude.
- Respectful of individuals situation, dealing with issues sensitively and confidentiality.
- The ability to work on own initiative and delegate.
- To work in a highly pressurised environment, with the ability to multitask and adapt to change.
- Ability to display empathy and sensitively towards relatives of individuals.
- The ability to work on own initiative and report as appropriate.
- This role operates outside of normal working hours, therefore commitment to flexibility is key to success.
- Full driving licence with access to a vehicle.

**Salary**: £27,000.00 per year
