The Older persons’ teams operate a single point of access model. Referrals are received from Crisis team, GP’s, care homes, Social Services etc, The IRS (Initial response Service) is the single point of access, and all the referrals are triaged through IRS before allocation to the various teams
The Community Treatment Teams manage referrals for frail, complex presentations of older people with functional and cognitive difficulties. There is not a specific age cut off between the old age and working age adult CTTs. It is based on the frailty and complexity of the person referred.
