Specialist Dietitian

Please refer to the attached job description for full duties of the role. To provide dietetic support to patients requiring nutrition support advice. This will include the assessment of nutritional status, calculation of nutritional requirements and prescription of diet therapy. To work as a member of the stroke service in community settings to facilitate a seamless transition of nutritional care to patients following stroke.

To provide specialist dietetic assessment, management and treatment of patients on HEF within the community setting To monitor and review the aims of nutritional care plans and recommend change or intervention to facilitate achievement of appropriate aims. To work collaboratively with the MDT to ensure appropriate dietetic intervention. To work as an autonomous practitioner within appropriate clinical and professional guidelines and as part of a team. To monitor and evaluate the effectiveness of dietary treatment including the recording of clinical outcomes and patient experience.

To take an active part in the training of trainee Dietitians. To contribute to the development of dietetic resources. To undertake an annual appraisal with the Senior Specialist Dietitian/Head of Dietetics. To identify personal training needs undertaking relevant CPD opportunities.

To help to provide cover for colleagues. To undertake any other duties appropriate to the grade/band as requested by the Head of Dietetics. To comply with the Trusts Vision, Values and Behaviours:
