The YOD service was first established in 2011, and across the past ten years has evolved into a multidisciplinary service providing clinical, nursing, psychosocial, and therapies support to people with a diagnosis of young onset dementia living within Cardiff and the Vale. Supporting carers and families is essential work. A core principle is that the YOD service provides continuity of care. The service engages with people from the point of diagnosis and remains involved across the entire course of an individual’s dementia progression.

The service is founded upon developing collaborative relationships of trust, understanding individual’s personal stories, bringing people together to enjoy mutually supportive social engagement activities, and being available to respond to often rapidly changing mental health, psychological, physical and social care needs.

In 2017 the service won the coveted Health Service Journal Award for Innovation in Mental Health Services.

The service is based at the Cariad Centre, Barry Hospital, which is alongside the service’s inpatient facility at St Baruc ward. This is a fantastic development opportunity to lead and support a team to work in the community broadening skills in holistic assessments of the needs of clients, carers and families.

The post holder will be expected to manager and care coordinate their own caseload of clients in the community, supporting outpatient clinic reviews and hospital discharge planning processes and undertaking reviews with clients in care home settings.

This is an exciting time to join the service as we forward plan and shape the service for post-pandemic work. You will be joining a highly dedicated and enthusiastic MDT where collaborative working both within the MDT and with allied health and social care partners, and third sector services is central to the ethos.

The ability to speak Welsh is desirable for this role; Welsh and/or English speakers are equally welcome to apply.
