Senior Clinical Practitioner for Looked After

This is a unique opportunity for a registered mental health professional, either a RMN or a HPC registered professional or equivalent registration to work in the Hive which is a partnership organisation working with the charity sector and the NHS. This post will suit an innovative thinker that wants to provide mental health support to young people who have been through and are in the care system. The post holder will enjoy working creatively with care leavers to help manage their mental health. The post holder will be embedded in a team that enjoys learning and treasures an environment that is compassionate to young people and staff.
The post holder will be a senior member of staff working closely with the management team of the HIVE, which consists of the Catch 22 Service Manager, Hub Manager and two Clinical Team Leads. One clinical team lead works for the Tavistock and Portman Trust NHS and the other Camden and Islington NHSTrust. They will contribute to and have some responsibility for the day to day clinical and operational management of the service, particularly as it pertains to looked after children, care leavers and refugees.
The focus of the post holder's work will be on young people who are under the care of Camden social services aged 16-25 and those who are refugees. Their caseload will consist of young people from these at risk groups. They may be asked to supervise young people’s workers who have looked after children, care leavers and refugees on their caseloads.
They will work under the supervision of the Clinical Team Leads; liaising with other agencies as appropriate developing strong relationships with the looked after teams in Camden and the refugees teams.
**The post holder will**:
- Have caseload of young people who are looked after young people, care leavers and refugees.
- Liaise with Camden CAMHS and Camden Adult Mental Health Services
- Liaise with teams that work with looked after children, care leavers and refugees
- Undertake risk assessment and risk management for relevant individual service users.
- Attend professional meetings, annual reviews and case conferences related to referred young people as appropriate.
- Understand the existing local services for young people using the project and contribute to decisions regarding appropriate referral onto other agencies.
- Contribute to multi-disciplinary and multi-agency work with young people as appropriate.
- Maintain a high standard of clinical record-keeping and maintaining good communication with colleagues.
- Observe personal duty of care in relation to equipment and resources.
- Practice within the legal and ethical framework set out by the NMC code of professional conduct and other relevant legislation such as the Mental Capacity Act, Mental Health Act and Safeguarding procedures.
- Uphold high professional standards at all time in line with Hub policies and Trust policies and guidance from the post holder’s relevant professional body.
Management and Leadership
- Use established measures to monitor clinical outcomes across the whole service.
- Lead on looked after children, care leavers and refugee mental health at the Hive and be a resource for the Hive Team
Teaching, training, and supervision
- Participate in an agreed programme of post-qualification training and professional development towards developing advanced knowledge and practice.
- Deliver training in The Hive way to other health and social care staff where appropriate, and internally to the staff team
- Deliver training and updates to Hive staff on working with looked after children, care leavers and refugees
Research, audit and service evaluation
- Utilise evidence-based literature and research to support evidence-based practice in the Hub.
- Carry out clinical audit and service evaluation within the service
- Carry out surveys and audits and produce concise, relevant and informative reports when required.
- Write case studies that might be useful for others to learn from
- Participate in young people engagement to build a young person led service.
- Receive regular clinical professional supervision from a senior professional colleague.
- Participate in an agreed programme of training and continuing professional development and maintain an active engagement with current developments in areas related to the post.
- Maintain up to date knowledge of legislation, national and local policies and issues of relevance to the service and service user group.
- Undertake such other duties and responsibilities, appropriate to the grade of the post, as may be agreed with the clinical manager from Camden and Islington Trust.
