The role will involve taking calls from new and existing community nursing patients; taking and documenting messages and working closely with triage nurses to prioritise calls.

You will be working in a dynamic office environment and have the ability to liaise with a variety of people both face to face or on the telephone. The successful applicant will be expected to be competent and confident in the use of IT systems, including Microsoft Office (Word, Excel and Outlook and Microsoft teams). Experience of entering accurate information onto databases is essential. Previous experience of clinical systems such as RiO, eCommunity, S2S and iPortal is also an advantage, however training will be provided to the successful applicant if required. The ability to travel between local Health venues is also essential for this role.

The successful applicant will be flexible and organised, have excellent telephone manner, be confident in speaking to patients and their families or carers; and have the ability to maintain composure when dealing with challenging situations. You will be expected to work autonomously and effectively prioritise your own workload in a busy environment.

**The main responsibilities are**:

- Call handling.
- Working closely with the triage nurses in the team.
- Liaising closely with the community nursing team to support effective handling of messages and referrals to the service.
- Liaising with partner organisations to ensure that all referrals or messages for the service are comprehensive and contain all relevant data to support the triage nurses to make safe and effective triage decisions.
- Recording calls and outcomes on the electronic clinical systems.
- Adding referrals to the electronic clinical system from our mailbox.
- General office admin duties (filing; photocopying; archiving etc).
