Senior Health Care Support Worker - Frailty Virtual Ward Lincolnshire Community Health Service NHS Trust Job summary There are 1 whole time equivalent Post 1 WTE for South East Business Unit - Covering Spalding, Market Deeping, Holbeach and Long Sutton and Boston populations This is an exciting opportunity to join the rapidly developing Frailty Virtual Ward which is a new service working with Lincolnshire Community Health Services, United Lincolnshire Health Trust and PCN’s to develop an integrated workforce. As part of our commitment to ensure people with the most complex health and care needs are proactively supported to live well at home or as close to home as possible. The Frailty Virtual Ward work in partnership with other agencies to provide holistic responsive and individualised support and assessment to patients across the county of Lincolnshire. Utilising a "What matters to me" approach that enables individuals to achieve their optimum physical, psychological and social wellbeing. Working as part of a multidisciplinary team contributing to improved health outcomes reducing unnecessary admissions to hospital and facilitating supported self-management. The role of Senior Health Care Support Worker will work across the County and community services to support provision of coordination to truly whole system integrated approaches to proactive care and support. ‘Lincolnshire Community Health Services NHS Trust is in the top 25 per cent of all community trusts for overall staff engagementin the 2021 NHS National Staff Survey. Main duties of the job This is a pivotal role as part of the Frailty Virtual Ward integrated workforce; working between the core team, primary and secondary care. **In this role you will**: Carry out assigned tasks, including direct nursing care and administrative duties as delegated by the Enhanced Practitioner and as part of a wider care team Support colleagues in the delivery of high quality care.
