**Initial Assessments**:
**Job Summary**:
**Key Responsibilities**:
- Conduct comprehensive initial assessments of new service users to determine their care needs.
- Develop individualized care plans in collaboration with service users and their families.
- Ensure all assessments and care plans comply with relevant regulations and company policies.
**Staff Supervision and Support**:
- Perform regular spot checks and supervisions to ensure staff are providing high-quality care.
- Provide ongoing support, guidance, and training to care staff.
- Address any performance or compliance issues promptly and effectively.
**Client Visits**:
- Visit clients regularly to ensure their needs are being met and adjust care plans as necessary.
- Address any concerns or issues raised by clients or their families.
- Provide hands-on care if required, covering calls to ensure continuity of service.
**On-Call Duties**:
- Be available on-call to address emergencies or urgent issues outside of regular working hours.
- Respond promptly to any on-call requests, providing support and direction as needed.
- **Administrative Duties**:
- Maintain accurate and up-to-date records of assessments, care plans, supervisions, and client visits.
- Report on the status of service users and staff performance to senior management.
- Participate in regular team meetings and training sessions.
**Experience**
- Previous experience as Care Coordinator in a healthcare or social services setting preferred
- Knowledge of medical terminology and healthcare procedures
- Proficient in Microsoft Office Suite (Word, Excel, Outlook)
- Excellent communication skills, both written and verbal
- Strong organizational skills with the ability to multitask and prioritize
- Ability to work independently as well as part of a team
Pay: £13.00 per hour
Expected hours: 37.5 per week
