Health Care Support Worker Homelessness Service

The Homelessness Service is a multi-agency service designed to support those experiencing homelessness or those who are vulnerably housed within CTM. The aim of the service is to encourage uptake of health interventions and engage the individual with the most appropriate health service.
We are looking for a dynamic and proactive Health Care Support Worker with an interest in supporting a vulnerable client group, experience within mental health and/or substance misuse who can support and carry out a range of psychosocial interventions.
The team consists of Nurses (both co-occurring and general health nurses), co-occurring support workers (employed by both health and Barod), RCT administrators and a team coordinator in addition to a part time medic and Trauma Informed Counsellors as employed by CTM Mind.
To deliver care as directed by registered nurses - undertaking various interventions as set out in the individuals Intervention Plan eg. anxiety management and harm reduction work.
Promote independence and encourage engagement with core health services. Liaison with services such as substance misuse services, GP's and Primary Care Mental Health Services.
Take an active role in team discussion, caseload management and clinical supervision.
Ensure effective communication with the individual and other services who may be offering support eg. housing, third sector services and/or criminal justice.
Provide education in respect of Overdose Awareness, Prenoxad administration and Blood Borne Virus testing (DBST).
Promote independence and work toward recovery.
Maintain confidentiality, ensure individuals records are up-to-date and maintain appropriate boundaries at all times.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
